Around here, the hole itself is rarely the actual problem. Water sneaks below the asphalt, the clay underneath turns to pudding, and then a loaded truck punches right through the top. Blame the water. It finds a hairline crack, settles into the clay under your pavement, and turns the base soft until the asphalt on top is basically a bridge with nothing holding it. One box truck later, the whole thing drops.

Compaction is where the cheap guys blow it. Dump too much material in a single lift and it never hits density, and by next summer traffic has rutted the whole patch back down.
Step 1. Somebody walks the whole property, probes the soft spots, and paints out every area that needs work.
Step 2. Saw cuts go in square and clean, past the failure instead of hugging the edge of it.
Step 3. Dead asphalt and soaked base material get dug out and hauled off site.
Step 4. Soft subgrade? Then we undercut it and rebuild with compacted stone before anything else happens.
Step 5. Tack coat hits every vertical face so the patch actually bonds rather than pulling away from the edges.
Step 6. Hot mix goes down in lifts, gets rolled or plated to density, and the edges get sealed up.
